OLMSTED FALLS, OHIO – Theresa Fazulak of Olmsted Falls won $500,000 playing the Ohio Lottery Scratch-Off, $500,000 Cashword.

After mandatory state and federal taxes totaling 28 percent, Fazulak will receive $360,000.

She purchased her winning ticket at Sunoco Mart, located at 871 N Rocky River Dr in Berea.
