QUAKER CITY, OH – Levi Troyer of Quaker City matched five numbers to win the Lucky for Life second tier prize of $25,000 a year for life in the November 19, 2021 drawing.

Levi chose to receive the cash option prize of $390,000. After mandatory state and federal tax totaling 28 percent, he will receive $280,800.

The winning numbers were 3-4-9-19-40. The Lucky Ball was 11. Troyer matched the first five numbers to win the game’s second-tier prize. He purchased his winning ticket at Valley View Service Station in Quaker City.
