Cleveland, OH — A Lucky for Life ticket sold at a Cleveland gas station has delivered a lifetime prize of $25,000 per year to one Ohio Lottery player, officials confirmed Tuesday.

The winning ticket for the April 28 drawing was purchased at Gas USA Harvard, located at 7020 Harvard Avenue in Cleveland. The ticket matched all five white balls — 16, 19, 20, 33, and 40 — missing only the Lucky Ball, which was 11, to win the game’s second-tier prize.

According to lottery officials, the winner selected their own numbers rather than using a quick pick option.
