Lucky numbers unlock life-changing payout as state’s first Millionaire for Life grand prize is claimed

CLEVELAND, Ohio — An Ohio lottery ticket purchased at a Cleveland grocery store has produced the state’s first-ever Millionaire for Life jackpot winner, resulting in a multimillion-dollar payout for a trust that claimed the prize this week.

The Ohio Lottery announced that The Angel Trust, represented by attorney Alan S. Levine, claimed the game’s top prize after matching all five winning numbers and the Millionaire Ball in the April 27 drawing.

The winning numbers were 4, 15, 19, 21 and 31, with a Millionaire Ball of 4.

The jackpot carried an annuitized prize of $1 million per year for life. Instead of receiving annual payments, the winner elected a one-time cash option worth $10 million.

After mandatory federal and state tax withholdings totaling 26.75%, the trust will receive approximately $7,325,000.

Lottery officials said the winning ticket was purchased at Giant Eagle #1263 on Edgecliff Avenue in Cleveland.

The prize marks the first jackpot winner since the multi-state Millionaire for Life game launched in February.
